The Champion Plan is a Medicare Advantage Special Needs Plan (SNP) available to residents with one or more chronic conditions. In addition to Original Medicare coverage, this plan has Part D prescription drug benefits. Members also benefit from a $0 copay for enhanced benefits like durable Medicare equipment and diabetes supplies.

  • Vision Benefit - Exam $0 copay, 1/yr, eyewear $100 credit every year (lenses/frames)
  • Falls Prevention Program covered for an assessment and subsequent instruction
