The Liberty no RX Plan is designed specifically for veterans of the U.S. Armed Forces and residents who do not participate in Medicare Part D.

Erickson Medicare Advantage Liberty (HMO-POS)

The Liberty Plan provides all the same benefits as the Liberty No Rx Plan but adds prescription drug coverage with a $0 premium.

Erickson Medicare Advantage Freedom (HMO-POS)

The Freedom Plan may be a good choice for residents who want additional certainty around out-of-pocket expenses. This mid-level plan is designed with extra coverage—including prescription drugs with no deductible and a ZERO copay for primary care visits.

Erickson Medicare Advantage Signature (HMO-POS)

The Signature Plan is our most complete plan, offering low to no copays on many services. This plan is ideal for residents who are looking for extra benefits like transportation, hearing aids, vision, podiatry, and prescription drug coverage with little or no deductibles.

Erickson Medicare Advantage Champion (HMO-POS C-SNP)

The Champion Plan is a Medicare Advantage Special Needs Plan (SNP) available to residents with one or more chronic conditions. In addition to Original Medicare coverage, this plan has Part D prescription drug benefits. Members also benefit from a $0 copay for primary care visits and for enhanced benefits like durable medical equipment and diabetes supplies.

The Guardian Plan is a Medicare Advantage Special Needs Plan (SNP) specifically for residents who live in or move to our long-term care setting. It’s a low-cost option that provides a great value for residents who need specialized care.
